The COVID-19 pandemic has exposed stark health inequities across the nation, causing payers and providers to take a closer look at how they can better incorporate SDOH into their operations. Our CEO and co-founder, Trenor Williams joined leaders from CVS Health, Geisinger and Humana to discuss what an effective program looks like and who’s getting it right during a virtual panel hosted by MedCity INVEST Digital Health earlier this month. The group discussed why taking a holistic approach and partnering with community organizations is key to standing up a successful social determinants of health program.
Emphasizing the importance of data and measurement, Trenor explained, “We create better visibility by aggregating information at the individual level to create risk scores. Providing that view helps healthcare providers better understand the risk individuals face based on the communities they live in and from there can better support their needs.”
For example, he pointed to efforts at health systems like Toledo, Ohio-based ProMedica, a Socially Determined client and emphasized how work needs to be done in conjunction with communities, not to communities.
